$349/YR | 3-12
- Two routine cleanings

- Complete x-ray coverage, as directed by your doctor
- Complimentary intraoral photos
- Two fluoride treatments, if indicated
- 20% off on all other dental services
MEMBERSHIP GUIDELINES
- Membership fee is due at time of enrollment, is valid for one year (12 months), and is non-refundable.
- If you are not an established patient (if you are new to our office or have only been seen for emergencies) there is a one-time fee of $50 upon enrollment.
- DapperVIP is NOT dental insurance.
- Membership cannot be combined with dental insurance.
- DapperVIP can only be used for services performed in our office.
- If the individual is getting Invisalign, they must remain a member until treatment is complete.
- Does not apply to costs covered by worker’s compensation or covered under auto or medical insurance.
